Identification and Emergency Information

Your child's details, parent and emergency contacts, doctor and dentist, and the people allowed to pick your child up. Complete both pages and sign on page 2. The facility fills in the last box.

Consent for Emergency Medical Treatment

Gives us permission to arrange emergency medical or dental care for your child, and lists any medication allergies. Write your child's name, your address and phone numbers, and any medication allergies, then sign and date it.
